Diabetic Retinopathy in the Context of Timira: Exploring the Ayurvedic–Modern Interface of Diabetic Retinal Changes

ABSTRACT
Diabetic retinopathy is a slowly progressive, sight-threatening complication of long-standing diabetes mellitus and remains one of the leading causes of preventable blindness among working-age adults worldwide. Modern ophthalmology explains it through a well-mapped sequence of micro vascular injury, ischemia and pathological vascularisation, while classical Ayurveda addresses similar territory through the concept of Timira. This category of progressive vision disorders linked to Prameha and its subtype Madhumeha. This article reviews the modern clinical understanding of diabetic retinopathy which outlines the Ayurvedic framework of Timira and its correlation with Madhumehajanya or Pramehaja Timira, and examines the pathophysiological parallels, therapeutic approaches and evidentiary limitations on both sides. It argues for a cautious, evidence-guided integrative model in which Ayurvedic care supports metabolic and lifestyle correction alongside, rather than instead of, standard biomedical retinal treatment.
